A Coru\u00f1a, Spain.<\/p>\n<p>RESUMO<\/p>\n<p>A artrose (<em>OA<\/em>) \u00e9 unha das enfermidades m\u00e1is prevalentes e incapacitantes do mundo desenvolvido. Nunha articulaci\u00f3n con <em>OA<\/em> todos os tecidos articulares vense afectados, pero a dexeneraci\u00f3n do cart\u00edlago articular \u00e9 o punto clave no desenvolvemento da patolox\u00eda. O cart\u00edlago articular \u00e9 un tecido conectivo especializado composto por unha pequena proporci\u00f3n de c\u00e9lulas, os condrocitos, que son responsables da s\u00edntese e o mantemento da matriz extracelular. Entre os factores que contrib\u00faen a progresi\u00f3n da <em>OA<\/em> at\u00f3pase un proceso inflamatorio persistente que resulta en niveis anormalmente elevados de citoquinas tales como a IL\u20111\u03b2 y la IL-6, de prostaglandinas, fundamentalmente a PGE<sub>2<\/sub>, e de metaloproteasas de matriz tales como a colagenasa 3 (<em>MMP13<\/em>) ou a estromelisina (<em>MMP3<\/em>). Tam\u00e9n se produce un incremento de especies reactivas de nitr\u00f3xeno e de os\u00edxeno, que someten \u00e1s c\u00e9lulas a un aumento de estr\u00e9s oxidativo e desencadean a morte celular dos condrocitos. Globalmente, todos estos mediadores act\u00faan destruindo a MEC de maneira irreversible.<\/p>\n<p>Actualmente non existe ning\u00fan tratamento que cure ou cando menos dete\u00f1a a progresi\u00f3n da <em>OA<\/em> e os tratamentos existentes (combinaci\u00f3n de terapia f\u00edsica e terapia farmacol\u00f3xica analx\u00e9sica e anti-inflamatoria) te\u00f1en efectos secundarios indeseados.<\/p>\n<p>Un tratamento en exploraci\u00f3n \u00e9 a balneoterapia con augas minero&nbsp;medici\u00f1ais sulfuradas, pero desco\u00f1\u00e9cese o mecanismo de acci\u00f3n polo cal alivian a sintomatolox\u00eda de patolox\u00edas como a <em>OA<\/em>. O sulfuro de hidr\u00f3xeno, que \u00e9 o componente principal destas augas, identificouse recentemente como un gas end\u00f3xeno (prod\u00facese no organismo a trav\u00e9s da&nbsp; enzimas <em>CBS<\/em>, <em>CTH<\/em> y <em>MPST<\/em>) e como un mediador da inflamaci\u00f3n. A relaci\u00f3n entre o cart\u00edlago e o sulfuro de hidr\u00f3xeno \u00e9 o aspecto clave no que se centrou este traballo. Comprobouse a presencia das enzimas de producci\u00f3n de H<sub>2<\/sub>S nos texidos da articulaci\u00f3n (membrana sinovial, \u00f3so subcondral e cart\u00edlago) e ao cuantificar a s\u00faa bios\u00edntesis atopouse que no cart\u00edlago artr\u00f3sico prod\u00facese menos H<sub>2<\/sub>S que no cart\u00edlago sano. Esto \u00e9 consecuencia, cando menos, dunha menor expresi\u00f3n de <em>MPST<\/em> neste tecido.<\/p>\n<p>Adem\u00e1is, utiliz\u00e1ronse diferentes dosis de dous compostos, o NaSH, que produce H<sub>2<\/sub>S de maneira instant\u00e1nea,e o GYY4137, que o fai gradualmente, para administrar H<sub>2<\/sub>S de maneira ex\u00f3gena a condrocitos e cart\u00edlago artr\u00f3sicos. Comprob\u00e1ronse que ambos, pero sobretodo o GYY4137, son capaces de revertir o proceso inflamatorio e reducir o estr\u00e9s oxidativo nas c\u00e9lulas, as\u00ed como previr o desequilibrio anab\u00f3lico\u2011catab\u00f3lico na matriz extracelular.<\/p>\n<p>Colectivamente os nosos&nbsp; resultados suxiren que a administraci\u00f3n ex\u00f3gena de sulfuro de hidr\u00f3xeno resulta beneficiosa na <em>OA<\/em> porque a articulaci\u00f3n artr\u00f3sica presenta uns niveis inferiores a articulaci\u00f3n sana.
